Position Summary:

The Community Engagement Specialist position is a liaison between Greater Health Now operations and communities within assigned region. Adequate GHN representation is expected at regional and local convenings both virtually and in person. The specialist will support existing contractual relationships and foster new connections to leverage GHN initiatives and support community needs. The role includes coordinating and facilitating various community gatherings including training and educational opportunities to support GHN mission of advancing population health. Generating and presenting ongoing report outs of community needs, resources, program outcomes and successes is vital in this role to help GHN stay apprised of gaps, resources and assets to better serve communities with intention.

Essential Job Functions:

  • Contribute to operations of the Greater Health Now regional hub and social care network.
  • Represent Greater Health Now in community meetings and events within the assigned region, virtually and in person.
  • Supports existing contractual relationships in assigned region and initiates and fosters new relationships and partners.
  • Coordinates and Facilitates convenings and development opportunities.
  • Connects community organizations to resources and opportunities supported by GHN and network partners.
  • Produce regular reports of regional activities and program outcomes.
  • Creates, manages, and updates Landscape Analysis data collection process in assigned region and collaborates with Data Analyst to ensure data elements are accurate and correctly represented.
  • Facilitates meaningful connections between Community Based Workers and Community Based Organizations.
  • Presents updates on community initiatives to operations team. Collects feedback and incorporates into projects.
  • Researches and educates on progress on Community Health Needs Assessment focus areas, and partners with organizations to understand Community Health Improvement Plan.
  • Communicate regularly with other managers and leadership within the organization.
  • Organize meeting preparation & facilitation- (design agendas, identify and secure content experts and speakers, document meeting notes and action items, and facilitate conversations and activities.)

Qualifications and Requirements:

Education:

  • High School Diploma or equivalent, required.
  • Associates degree or trade school, preferred.

Experience:

  • Minimum of three years of population or public health or social service experience.
  • Bilingual English /Spanish, highly preferred.

Knowledge, Skills and Abilities:

  • Experience in program development/management, grant funded programs or government contracts, preferred.
  • Reliable and efficient work ethic.
  • Ability to function using their own individual initiative and as a team member on a multidisciplinary team.
  • Effective communication and active listening skills with coworkers, community organizations, community-based workforce, and community members.
  • Understands community-clinical linkages and Health Related Social Needs.
